Is it true that a radio station won't be listed in the Nielsen Ratings Book, if that station doesn't subscribe to their service?
Has this happened before to anyone who's worked in radio?

Yes, this has been true for many years now. You have to "pay to play!" There are some stations, like WHMI in Howell/Livingston County, who make a considerable profit, without paying to subscribe to either the Detroit Metro or Lansing Nielsens.

So, a Detroit station, for example, might have a relatively large audience, and doesn't show up in the book, if they don't subscribe. And, the agencies know this? How can they justify this to their clients?

Incorrect. If you don't pay, you don't have access to the information, to use for sales, etc. And you aren't listed in All Access, Radio Ink, etc. But if you subscribe, you see everyone's numbers. Agencies subscribe, thus they see all the numbers. Everyone who has listening gets rated.
